An incident is filed at the pool where it happened, with what was done about it and who owes the next step. A rescue is its own record. And when a reading or a check fails, the incident can start from it with the evidence already attached.

A scraped knee and a rescue are not the same paperwork, so they are not the same record. Both are filed at the pool where they happened, and both carry who is responsible for what happens next.

Pick the pool and, if you use them, the area within it. Categorise it — safety, maintenance, equipment, chemical or other — and set a severity from low to critical.
Who reported it, the witnesses, the actions taken, and up to ten photos.
Assign it, set a follow-up date, and move it from open through in progress to resolved.
A rescue records the guard, the pool and area, the save performed, whether EMS was called, and whether a manager still owes a follow-up.
An out-of-range chemical reading or a failed maintenance check becomes an incident in one step, with the reading or check linked as evidence and the category set for you.
The offer to open an incident appears right where the bad reading or failed check is recorded, while the person who found it is still standing there.
Readings and checks are linked to the incident itself, and must come from the incident's own pool or be a site-wide check at that site — a reading from another pool cannot be attached by mistake.
Rescues have their own permissions. Someone who logs a cracked tile does not, by that alone, see who was pulled from the water.
A full report of any injury or illness incident occurring at a swimming pool shall be reported by the pool owner/operator to the permit-issuing official within 24 hours of its occurrence, and a notation recorded in a log book.
